چکیده

Possibility of hidden damages with temporary uterine artery occlusion device Sir, I read the article of Vilos et al. (2006) with interest, but I need to give some opinions about the use of this Doppler-guided transvaginal clamp. First, it is possible that the pelvic ureter will be occluded (Al-Awadi et al., 2005) when the transvagi-nal clamp closes. In this case, the ureter could be damaged, and the effects of this lesion could be fibrosis or stenosis. Probably, if the control could be made at the same moment as the clamp placement, the clamp could be relocated before causing occlusion. In this way, the ultrasound control referred by the authors does not result in a secondary structural injury after 6 h compression. Second, The authors stated that with this procedure the effect of ovarian dysfunction may be avoided, but this phenomenon seems be multiple. In approximately 10% of cases, the ovary circulation depends mainly on the uterine artery. If the uterine artery compression is performed in this circumstance, the lack of ovary irrigation could produce an ovary dysfunction, as embolization or hysterectomy cases were seen (Ryu et al., 2003). Cases of ovary dys-function were reported after gross haemorrhages or uterine devascularization (Roman et al., 2005). In both examples, the lack of ovary irrigation could have been the main cause of secondary ovary failure. Regardless of this point, the authors have not considered that the deficient ovary irrigation after 6 h uterine artery occlusion will be the cause of secondary ovary failure. This is particularly important in patients over 35 years old who have more possibility of ovarian failure, and also those patients who consider a future pregnancy. In order to establish the risk on this point, it would be useful to measure FSH and estradiol after and before the utilization of transvagi-nal clamp. I completely agree with the authors that further studies are needed; maybe these comments will be useful to complement this novel study. Background and Objective: Recent studies suggest that sub-lethal ischemia protect the brain from subsequent ischemic injuries. This study was an effort to identify and shed light on the nature of changes in the blood brain barrier permeability and brain edema. Materials and Methods: Rats were divided into four main experimental groups, each of 21 animals. The first group acted as a model of isc...

Collateral development after carotid artery occlusion in Fischer 344 rats.

Mortality following permanent occlusion of both common carotid arteries decreases as the time between the first and second occlusions increases in Fischer 344 rats. Our goal was to examine the possibility that collaterals develop after unilateral carotid artery occlusion. During temporary occlusion of both carotid arteries in nine ketamine-anesthetized male rats, mean +/- SEM blood flow in both...

Ulipristal acetate before high complexity endoscopic (hysteroscopic, laparoscopic) myomectomy – a mini-review

Uterine myomas (fibromas, leiomyomas) are the most common tumours in women, and their clinical signs and symptoms are presented by 25-40% of patients with these benign tumours. According to current guidelines, the armamentarium for myoma management consists of: medical therapy (GnRH, SPRMs), non-surgical alternatives including uterine artery embolisation (UAE), vaginal temporary occlusion of ut...

Conservative treatment of a cervical twin pregnancy with uterine artery embolization.

Cervical pregnancy is a rare form of ectopic pregnancy. Its treatment has been described by different authors. We report our successful experience of a cervical twin pregnancy that was diagnosed by transabdominal and transvaginal ultrasound and confirmed by magnetic resonance imaging. To preserve fertility, our patient was treated by a bilateral hyperselective uterine artery embolization follow...

Management of symptomatic fibroids: conservative surgical treatment modalities other than abdominal or laparoscopic myomectomy.

Approximately 50% of women of reproductive age have fibroids, and at least 50% of these women have significant symptoms. However, until 15 years ago, the only surgical options available were hysterectomy and myomectomy, and as yet there are no proven effective long-term medical therapies. Fortunately, the past decade has witnessed the emergence of highly sophisticated diagnostic and therapeutic...
